How can I get the paperwork of his rights?

Ok, just a quick question.. I need to get the paper work of my daughter's biological father's rights, served to him if possible... I mean, he isn't on the birth certificate, doesn't have any proof of DNA, nothing, NOTHING legal has been done.. but I'm going to place my soon to be husband on the Birth Certificate, and I want to ask him to sign his rights over. I don't want need attorney for anything else but this, how much would it cost? I know it depends on who the lawyer is, but just about? Can I get it from the courthouse? I know I can't get it from the department of human services, because she is on medicaid and they aren't going to help me get him to sign his rights over, because they want to go after him for child support.
